Release and Waiver of Liability

PLEASE READ CAREFULLY. THIS IS A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS. BY SIGNING IT, YOU GIVE UP THE RIGHT TO SUE THE RELEASED PARTIES FOR INJURY, DEATH, OR PROPERTY DAMAGE, INCLUDING CLAIMS CAUSED BY THEIR NEGLIGENCE.

In consideration for being permitted to participate in the annual community 5K run, I, Jordan Reyes, on behalf of myself and my heirs, executors, administrators, and assigns, agree as follows:

1. Assumption of Risk

I acknowledge that the annual community 5K run involves inherent and unforeseeable risks, including but not limited to muscle strain, sprains, falls, dehydration, weather-related illness, and contact with other participants, any of which could result in serious injury, illness, property damage, or death. I voluntarily assume all such risks, known and unknown.

2. Release of Claims

TO THE FULLEST EXTENT PERMITTED BY LAW, I RELEASE, WAIVE, AND DISCHARGE COMMUNITY ATHLETICS ASSOCIATION, its officers, directors, employees, agents, volunteers, and sponsors, and the owners of any premises used for the annual community 5K run (the "Released Parties"), FROM ANY AND ALL CLAIMS FOR INJURY, ILLNESS, DEATH, OR PROPERTY DAMAGE ARISING OUT OF OR RELATED TO MY PARTICIPATION IN THE ANNUAL COMMUNITY 5K RUN, INCLUDING CLAIMS ARISING FROM THE ORDINARY NEGLIGENCE OF THE RELEASED PARTIES. This release does not apply to claims arising from the gross negligence, recklessness, or willful or in

3. Indemnification

I agree to indemnify the Released Parties against third-party claims to the extent caused by my own negligent or wrongful acts or omissions while participating in the annual community 5K run.

4. Medical Authorization

I authorize the Released Parties to secure emergency medical care for me if I am unable to consent at the time of need, and I am responsible for the cost of that care.

5. Representations

I am at least 18 years old (or the age of majority where I live), I am physically able to participate, and I will follow the rules and instructions of the Released Parties.

6. Governing Law and Severability

This Release is intended to be as broad and inclusive as permitted by the laws of the State of Illinois, which govern it. If any part of this Release is held invalid or unenforceable, the rest will continue in full force and effect.

I HAVE READ THIS RELEASE, UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT VOLUNTARILY.
